• Our membership subscription plan is not insurance and is not a qualified health plan under the Affordable Care Act.
• Annual Membership Fee must be paid in full before benefits begin.
• Annual Membership Fee is non-refundable and non-transferable.
• Membership will continue for one year from date of enrollment, and will renew on the anniversary date.
• May not be used in conjunction with any other dental insurance plan.
• Our regular fees are subject to revision annually. 
• We reserve the right to change the membership fee or any other terms annually.
• Services provided by referral specialists are not subject to discount.
• After-Hours Office Visits are not included.
• Sales of products (e.g. Zoom bleach and prescription fluoride gel) are not included.
